Submitting a nomination

Thank you for your interest in nominating, and we look forward to your nomination in 2018.

In 2018 a nomination requires:

  • A completed nomination form (including nominator and nominee statement)
  • A maximum four page CV
  • One portrait photograph of at least 300 dpi resolution

The submission process will be conducted through an online submission process this year. The form will remain largely the same.

If you are unable to complete the online form, please contact us before submitting using the pdf form.Note: this is a form to fill in, it will attach to an email as an .fdf file which we are able to open. If possible please use this format rather than a scanned pdf.

Please read all the eligibility, selection and other background before submitting a nomination.

Nominations will open at 9am on Monday 26th February and close at 11:59 pm on Monday 9th April.
ALL nominations must be submitted via the online form. If you are unable to complete online, please contact us before submitting using the pdf form.
