
Submitting a nomination
Nominations should be either emailed (or posted) to the addresses below for each state/territory by the respective closing dates. Email is preferred.
For any posted nominations, two copies of each nomination should be enclosed and should be printed single-sided and securely fastened (but not stapled or bound) along with enclosures
No faxed nominations will be accepted.
